Prescription Refills

In our ongoing effort to make your pet's health care as convenient and easy as possible, you can now request a refill for your pet's prescription by submitting the following form. Please be sure to fill in all the requested information. The prescription refill must be approved by a doctor. If the pet you are requesting the presciption for has not been examined by a doctor at Griffin Animal Care within the past 12 months or has no proof of current Rabies vaccination, the request will be denied. Any tests required must be completed before the prescription will be filled (this includes heartworm testing, drug levels, etc).

We will notify you via email or phone when your pet's prescription is approved and ready to be picked up. If you would prefer to have the prescription mailed to you, please mention this information in the additional information area, we would be happy to mail the prescription to you. Griffin Animal Care is not responsible for any medications that are lost in the mail.
